Incident Report Form for Discrimination, Harassment, Sexual Misconduct


This form is used to refer an incident involving an alleged violation of General Order 16 (GO 16) VMI's Discrimination, Harassment, and Sexual Misconduct Policy. 

This form may be used for anonymous reports; reporting individuals may omit their information when requested. Please note: While VMI makes every effort to address/investigate anonymous reports, we may be limited in our ability to do so without sufficient information.
